When registering for Camps and Clinics or Lessons, you agree to the following:

By registering for these training programs I confirm that I have no knowledge of any physical impairment that would affect this player from participating in the program. By completing the registration for this training, I agree that in case of accident during lessons, camps or clinics, I authorize the Pure Performance staff to act for me in securing medical treatment, and release the staff of any liability.
